Lodge Complaint

To lodge a complaint with the Ombudsman, please fill in the form below.

Section 1 requires details of your complaint. Section 2 requires your personal contact details. Section 3 requires proof of your identification, and Section 4 (Optional) seeks information on your employment.

Before you make a complaint to the Ombudsman, you must first try to resolve it with the Ministry, Department or Agency (MDA) concerned. The Ombudsman may decide not to investigate unless you have attempted to do this. If the MDA has a complaints process, you should follow it. If not, you should first contact the MDA directly with your complaint.

If you have material regarding your complaint in electronic format, be prepared to upload it below. If not, you may submit supporting material by post or in person to the Office of the Ombudsman. You will also need a photograph or scan of your Personal Identification document to upload, or a copy to deliver with any physical supporting material.

3. Proof of Identification

Your Proof of Identification will only be used by the Ombudsman to verify your identify. Details submitted will not be passed to any third party and will be deleted following verification.


You must provide a photograph or scan of your Proof of Identification document. For passports, only the ID page is required.

If you are sending or delivering physical copies of your Proof of Identification, please label them clearly with your personal details.


4. Employment (Optional)

This information is collected for statistical purposes only. Details submitted will not be passed to any third party.


Important Notes

Be sure to inform the Ombudsman if your contact details change. You may do this by email, post or calling 0800 662. If you fail to respond to any communications within 21 days, the Ombudsman may close your file.
